DOT PHYSICALS
DOT regulations require that the following conditions be well controlled and certainly information from your personal physician is required before we can complete your physical.
Please note that once we start your physical, we must complete it, even if you don’t pass. Your fee can not be refunded and you will be charged for another visit if you need to return on a later date with the proper documentation.
BEFORE YOUR VISIT
We require that you have a well-controlled blood pressure (<140/90 mm Hg)
BRING THE FOLLOWING ITEMS:

A list of all Medications (prescription and non-prescription)

Your glasses or contact lenses (if you wear glasses to drive)
IF YOU HAVE A HEART CONDITION:
You must have a clearance letter from your cardiologist dated within the year.
IF YOU HAVE SLEEP APNEA:
You must have a clearance letter from your SLEEP APNEA DOCTOR dated within the year.
IF YOU ANY SERIOUS MEDICAL CONDITIONS:
You must have a clearance letter from your SPECIALIZED (e.g., neurologist) DOCTOR dated within the year.
